As we strive to provide valuable services to youth with special health care needs and disabilities, it is important to know what resources and tools are available to help parents and guardians prepare their youth for the transition to adult life.

During this webinar you will hear from the Kansas Department of Health and Environment (KDHE), Children and Youth with Special Health Care Needs (CYSHCN) and the Kansas University Center on Developmental Disabilities (KUCDD) about a model developed to support youth health care transition.

Parents and families will:
~ Hear about a multi-system approach to health care transition that centers around youth and families.
~ Get tools to help empower youth to be independent.   
~ Learn about resources available to families that can help throughout the transition process.
~ Understand how these activities can support successful transition to adult life.
